When creating an assessment plan, there are certain typical components - The document linked here was created by Dr. Marilee Bresciani (Ludvik), and can serve as a worksheet to document the assessment planning process, including the collection of data for reporting. Here is a list of the eleven typical components of an assessment plan and report:
- Program name
- Program mission or purpose
- Program goals
- Outcomes
- Planning for delivery of outcomes
- Evaluation methods/tools (sometimes called measures)
- Implementation of assessment process

- Reflection, interpretation, decisions and recommendations
