Assessment Plan Components

When creating an assessment plan, there are certain typical components - The document linked here was created by Dr. Marilee Bresciani (Ludvik), and can serve as a worksheet to document the assessment planning process, including the collection of data for reporting. Here is a list of the eleven typical components of an assessment plan and report:

  • Program name
  • Program mission or purpose
  • Program goals
  • Outcomes
  • Planning for delivery of outcomes
  • Evaluation methods/tools (sometimes called measures)
  • Implementation of assessment process
  • Results
  • Reflection, interpretation, decisions and recommendations
  • Documentation of process/feedback
