Recreation/Sport Management Internship Procedures

The following is a step-by-step outline of the procedures for a Recreation/Sport Management Internship (HPHE 4990) offered through the WMU Department of Human Performance and Health Education.
- Download the Internship Check Sheet to track your application.
- Print and READ the Internship Requirement Information Form. This is important. It explains the internship and by checking that off, you acknowledge that you have read it.
- Complete the application for the internship you have selected. The forms are in PDF. You can write and save them.
- Recreation Management Application
- Sport Management Application
- Make an appointment with Dr. Cork to review all the procedures, guidelines and your internship. Please come prepared. Bring all documentation. If you fail to bring documentation, your registration and internship will be delayed.
- The following are the forms required to successfully complete your internship. Please complete these forms and keep them in a file (or electronic file) necessary to compile your final report:
- Weekly logs
- Bi-monthly evaluation
- Mid-Term Evaluation (complete after 300 hours)

Internship Portfolios
All reports necessary for the internship portfolios are available electronically both as writable PDF or Microsoft Word at:

You may choose to compile a report (similar to a paper) and turn your internship report in as a paper, or you might choose one of the following programs. These programs are e-portfolio programs and often provide free space for a Â鶹´«Ã½ to create a portfolio:
There are plenty of user friendly e-portfolio sites available (usually free). You can use any of them. Choose one that you like and that is easy for you. As of the beginning of Fall 2017, any submission must be electronic. If you have questions, please contact Dr. Colin Cork
Getting started
How to find an internship:
- Meet with WMU Career and Student Employment Services
- Connect with WMU alumni (LinkedIn)
- Online search (, )
- Check WMU Athetics or Facilities
- Utilize previous employment (former supervisors, managers, co-workers)
- Ask fellow Â鶹´«Ã½s
Internship requirements:
Print and READ the Internship Requirement Information Form. This is important. It explains the internship and by checking that off, you acknowledge that you have read it.
Registering with Dr. Cork:
- You will be sent an email giving you permission to register for the course
- *Although your internship may take longer than a semester, you only need to register once
- Once registered, you will have access to the HPHE 4990 site on eLearning
Using eLearning:
- Submit the name of your internship site supervisor, their contact information, and your expected job responsibilities (in writing)
- Familiarize yourself with the necessary forms for your internship (log sheets, evaluations and reports)
During your internship
Things to do:
- Every 2 weeks, you must submit your log sheets to the eLearning site
- When you submit the logs, it's your responsibility to contact Dr. Cork saying they've been submitted.
- With internship supervisor, complete
- By yourself, complete a (after 300 hours)
- Note: Hold on to your logs and evaluations as they will be necessary to compile your final report.
After your internship
Things to do:
- Complete the final evaluation of internship form
- Submit your final report
How to create your final report:
- You may choose to compile a report (similar to a paper) or create a portfolio
- Students often use

- The final report is submitted to Dr. Cork using the eLearning site
