Exam Submission Form

For each exam that Â鶹´«Ã½(s) will take at DSS, complete this form after communicating with the Â鶹´«Ã½s. For Â鶹´«Ã½s to receive test accommodations within DSS, the professor or instructor must have received the "Semester Request Review Decision" email, indicating test accommodations.

Exams sent to DSS without this form will not be administered and Â鶹´«Ã½s are directed to communicate with the professor or instructor. DSS prefers that the form and exam are submitted 48 hours or more prior to the test date, in case DSS staff members have questions.

DSS takes the security of each test seriously. Tests are secured within a locked space. DSS employees escort Â鶹´«Ã½s to testing rooms and verify the correct test is being administered. DSS employees view testing rooms via a monitor that displays a live feed from a camera in each room. Only items authorized by professors or instructors are permitted in the testing room. 

DSS exam proctoring hours are Monday to Friday, 8:30 am to 5 pm, and all exams must be completed by 5 pm. Final exams at DSS are proctored at 8 am, 11 am, 2 pm and 5 pm Monday to Thursday of finals week. The Â鶹´«Ã½ may schedule Friday exams with the Testing Accommodations Coordinator with faculty approval.

Please Note: Submission of this form does NOT create an appointment for a Â鶹´«Ã½. Students must schedule their own appointments (at least 7 days in advance) through DSS-Accommodate.

Approved Testing Materials

In the case of suspected academic dishonesty, the DSS procedure is to take the material in question out of the testing room (e.g. cell phone, unapproved note sheet, other technology) and then take the exam from the Â鶹´«Ã½ temporarily. A copy of the exam is provided to the Â鶹´«Ã½ to finish the exam, while DSS staff members contact the professor or instructor. It is the within the professor's or instructor's discretion on how to proceed. DSS can provide digital files of the surveillance for the professor or instructor if they provide an external hard drive or large USB drive.
